Benzoyl peroxide and moisturizer: which goes first?

Benzoyl peroxide dries the skin, and a moisturizer is the simplest way to keep using it. The American Academy of Dermatology (AAD) names benzoyl peroxide first in its list of acne treatments that "tend to dry and irritate the skin," and says "using a moisturizer every day can help your skin tolerate these medications." Why benzoyl peroxide needs a moisturizer

The FDA label on every store benzoyl peroxide product warns that "excessive drying of the skin may occur" and lists "redness, burning, itching, peeling, or possibly swelling" as possible reactions. Prescription labels show how common dryness is: in five trials of a clindamycin and benzoyl peroxide 1.2%/5% gel, 15% of people had mild dryness during treatment and 1% moderate dryness, against 6% before treatment began.

Dry skin can also feed acne. The AAD explains: "When your skin becomes dry, your body makes more oil. The extra oil can clog your pores, which may lead to more breakouts. The right moisturizer can prevent your skin from becoming dry and irritated."

What the studies found

Three studies tested benzoyl peroxide together with a moisturizer. None was large, and they tested routines rather than moisturizer alone.

The 2023 trial is the strongest. Its authors wrote that the gel "damaged the skin barrier," measured as water loss through the skin, and that the ceramide cleanser and moisturizer reduced how often and how badly dryness, redness and scaling appeared, while "more quickly resolving barrier damage and restoring function." A dermatologist quoted by the AAD describes ceramides as "waxy lipid molecules that help hydrate skin."

The AAD also reports, from studies where people used an acne medicine, a cleanser and a moisturizer chosen by dermatologists, that "most patients see less acne between 4 and 8 weeks after beginning to use an acne friendly moisturizer."

How to choose a moisturizer

The AAD says to look for one of these words on the container: "oil-free," "non-comedogenic," or "won't clog pores." It says these mean "the moisturizer is unlikely to cause acne." Other practical points:

• Ceramides have the most study alongside benzoyl peroxide gels, from the trials above.
• Fewer irritants. A dermatologist quoted by the AAD advises avoiding products with "alcohol, alpha-hydroxy acid (AHA), and fragrance." Skinli's plan asks for a "light moisturizer."
• No extra acne ingredients. A moisturizer with salicylic acid or other acids adds a second acne medicine. The FDA label warns that "skin irritation and dryness is more likely to occur if you use another topical acne medication at the same time."
• Sunscreen in the morning. The benzoyl peroxide label says to "avoid unnecessary sun exposure and use a sunscreen." A daytime moisturizer with SPF, or a moisturizer followed by sunscreen, covers this.

Which goes first

The FDA label says to "clean the skin thoroughly before applying this product." Most routines put benzoyl peroxide on clean, dry skin and moisturizer on top. Skinli's 12-week plan does this at night:

1. Wash with a gentle face wash, a coin-sized amount.
2. Wait until your face is fully dry. The plan's reason: gel on wet skin stings more.
3. Apply a pea of benzoyl peroxide 2.5% gel over the whole area that gets pimples.
4. Wait 5 minutes. If you use adapalene 0.1% gel, it goes on next.
5. Apply a blueberry-sized amount of light moisturizer.

In the morning, the plan uses face wash, moisturizer and sunscreen SPF 30, with no benzoyl peroxide.

The sensitive-skin start. Some people put moisturizer on first. NICE, which writes acne guidance for England, suggests starting with "alternate-day or short-contact application (for example washing off after an hour)" to reduce irritation. Skinli's plan does both for people who say their skin is sensitive: for the first 2 weeks it puts moisturizer on before the gels, because "a soft layer under the gel means less sting," and washes the gels off with water after 1 hour, then moisturizes again. The 2012 study above also applied moisturizer before the gel.

No study Skinli found compared moisturizer-first and moisturizer-last directly. If your skin is comfortable, gel first and moisturizer second follows the label. If it stings, moisturizer first is a reasonable way to start.

How much, and how often

The AAD says to apply moisturizer "when your skin feels dry" and recommends "using it every day when your skin feels dry." With benzoyl peroxide, that usually means every day. Skinli's plan uses a blueberry-sized amount of light moisturizer twice a day: in the morning under sunscreen, and at night as the last step after the gels. If one area peels more than the rest, add more moisturizer to that area, and keep the benzoyl peroxide itself to a thin, even layer, as the label directs.

Moisturizer and a wash

If you use a benzoyl peroxide wash instead of a leave-on gel, moisturize right after rinsing and patting dry. The AAD suggests applying moisturizer after washing, and says: "After washing your face, leave some water on your skin. Then apply moisturizer." Moisturizer with prescription gels

MedlinePlus advises people using clindamycin and benzoyl peroxide gel to "ask your doctor or pharmacist to recommend a moisturizer to keep your skin soft during treatment." MedlinePlus's page on benzoyl peroxide alone says not to put "cosmetics, lotions, or other skin medications" on the treated area unless your doctor tells you to. The AAD recommends a daily moisturizer with benzoyl peroxide. If you are unsure, ask your pharmacist which moisturizer to use and when.

Moisturizers that contain benzoyl peroxide

DailyMed lists a few lotions sold as moisturizers that contain benzoyl peroxide. One 2.5% "moisturizer" carries the full acne Drug Facts label: use "a dime-sized amount to cover the entire affected area with a thin layer one to two times daily," test it on small areas for the first 3 days, and expect it to bleach "hair or dyed fabrics." These are acne medicines. If you also use a benzoyl peroxide gel, a benzoyl peroxide moisturizer doubles the medicine, so pick one or the other.

When moisturizer is not enough

If your skin still burns, peels or feels raw with a moisturizer, the label says to "reduce application to once a day or every other day," and irritation "may be reduced by using the product less frequently or in a lower concentration." Start at 2.5%, as the strengths guide explains. The NHS says common side effects "usually improve after a few days, or if you use the gel or gel wash less often." When to see a doctor

See a doctor if irritation is severe or does not settle, if your acne has not improved after 12 weeks, or if you have deep, painful acne.
