Retinoid with benzoyl peroxide: can you use both?

A retinoid and benzoyl peroxide are the two acne ingredients that the American Academy of Dermatology (AAD) guideline recommends most strongly, and doctors often combine them. People still ask whether the two cancel each other out, which one goes on first, and whether retinol counts. This guide answers those questions from the 2024 AAD guideline, NICE, NHS pages, MedlinePlus and drug labels, and it says plainly where the sources are silent. Why the two are paired

The 2024 guideline gives a strong recommendation, on moderate certainty evidence, for both benzoyl peroxide and topical retinoids. It also recommends combining treatments: "When managing acne with topical medications, we recommend multimodal therapy combining multiple mechanisms of action." On the two together, it recommends a fixed dose combination of a topical retinoid with benzoyl peroxide, again with a strong rating, and reports that at 12 weeks, in 3 randomized trials, a greater proportion of patients treated with combined benzoyl peroxide and a topical retinoid achieved success on the Investigator Global Assessment than patients treated with vehicle. Vehicle means the same product without the medicine, and the assessment is a clinician's overall rating of acne severity. The guideline adds that fixed-dose combinations facilitate adherence.

NICE reached a similar view. Its rationale says the evidence showed that combinations of topical treatments that included benzoyl peroxide, clindamycin and/or a retinoid (adapalene) were overall more effective than any of them used as topical monotherapies, for any severity of acne. The AAD describes the two working together: benzoyl peroxide reduces acne-causing bacteria and works best with another treatment, and a retinoid helps to unclog pores.

NICE names the adapalene and benzoyl peroxide fixed combination as a first-line option for acne of any severity, at either 0.1 percent adapalene with 2.5 percent benzoyl peroxide or 0.3 percent adapalene with 2.5 percent benzoyl peroxide, applied once daily in the evening. Using two separate products

Not everyone uses a fixed combination. The AAD says a retinoid and benzoyl peroxide "means buying two different products, but this combination works well in skin of color." On back acne it says that using adapalene 0.1 percent gel along with benzoyl peroxide can improve your results, because the retinoid helps unclog pores, which helps the benzoyl peroxide work better, and that dermatologists recommend applying adapalene after you shower or before going to bed, daily. Its page on blackheads and whiteheads says to use a retinoid and also a benzoyl peroxide wash.

The sources disagree on timing. An NHS hospital leaflet on adapalene says you can use benzoyl peroxide, clindamycin or erythromycin treatments while using adapalene, at different times during the day, for example adapalene in the morning and the other acne treatment at night, or the other way round. The AAD and MedlinePlus advise evening or bedtime use of retinoids. MedlinePlus says to apply benzoyl peroxide at a different time of day than tazarotene. No source read for this guide gives a layering order for two products applied together.

The NHS page on benzoyl peroxide adds a general rule: "Only use benzoyl peroxide together with other acne treatments if your doctor or pharmacist has told you that you can." The over-the-counter Differin Epiduo label says irritation is more likely in the first four weeks of use, and if using more than one topical acne medication at a time.

Tretinoin needs care

The guideline draws one distinction that matters here. It says some tretinoin formulations should be applied in the evening because they are sensitive to light, and should not be applied with benzoyl peroxide so that the tretinoin does not break down and stop working. It says tretinoin microsphere formulations, adapalene and tazarotene lack similar restrictions. MedlinePlus says that while using tretinoin you should not use other topical medications, especially benzoyl peroxide, unless your doctor directs you to. The Retin-A Micro label says benzoyl peroxide, sulfur, resorcinol or salicylic acid used with it may increase the risk of dryness, redness and peeling.

The formulation therefore decides the answer. Twyneo is a tretinoin and benzoyl peroxide cream made as one product, and the Arazlo tazarotene lotion label says to avoid using it at the same time as oxidizing agents such as benzoyl peroxide, or to apply each at different times of the day. Ask the prescriber which product you have.

What about retinol

No guideline read for this guide names retinol, so none gives a rule about retinol with benzoyl peroxide. One 2022 study followed 33 people with mild to moderate acne who used benzoyl peroxide 2.5 percent in the morning and a retinol moisturizer in the evening for 12 weeks. It reported a significant reduction in total acne count. The abstract mentions no comparison group, so it cannot show what retinol added. Strength, irritation and bleaching

The FDA rule for over-the-counter acne drugs allows benzoyl peroxide at 2.5 to 10 percent. The AAD says that while you will find products that contain up to 10 percent, it is best to start with a product that contains 2.5 percent. The two together irritate more than either alone. The Epiduo Forte label says redness, scaling, dryness and stinging or burning may be experienced, most likely during the first four weeks, mostly mild to moderate, and usually lessening with continued use. The AAD says that possible side effects of these combination medications include dryness, irritated skin, peeling, burning and worsening acne, and that applying more than your dermatologist recommends will not clear acne faster.

Benzoyl peroxide bleaches. The Epiduo Forte label says to wash hands after application because the gel may bleach hair or colored fabrics. NICE lists bleaching of hair and fabrics for the adapalene and benzoyl peroxide combination, and an AAD page says benzoyl peroxide will bleach clothing, sheets, pillow cases and towels.

How long to use the combination

NICE offers a 12-week course of a first-line option and says to review it at 12 weeks, checking whether acne has improved and whether there are side effects. It says positive effects can take 6 to 8 weeks to become noticeable. The AAD gives similar advice for a new product: "Use that product for 6 to 8 weeks," and during this time, do not add another product and do not stop using the product you started with. The NHS page on benzoyl peroxide says it usually starts to work within 4 weeks, and it says to speak to a pharmacist or see a GP if acne gets worse or does not improve after 12 weeks.

Acne can return when treatment stops. The AAD says that even when you see clearing, you will want to keep using the acne treatment, which helps prevent new breakouts, and that dermatologists may recommend a topical retinoid as a maintenance treatment. NICE says maintenance treatment is not always necessary, and that a fixed combination of topical adapalene and topical benzoyl peroxide can be considered as maintenance for people with a history of frequent relapse.

Ages and pregnancy

Age limits differ by product, as the table shows. On pregnancy, NICE lists adapalene with benzoyl peroxide as not for use during pregnancy and says topical retinoids should not be used during pregnancy and when planning a pregnancy. When to see a doctor

If acne is painful, cystic, getting worse, or has not improved after a few months of steady care, see a doctor. NICE says that if acne fails to respond adequately to a 12-week course of a first-line option, another option should be offered.
