Yes, you can use niacinamide with tretinoin, and ask the doctor or pharmacist who gave you the tretinoin before you add it. In a 2006 study, women used a moisturizer with niacinamide and two other vitamins twice a day for 10 weeks and started tretinoin cream 0.025 percent 2 weeks in. The authors concluded that the moisturizer made the early phase of tretinoin treatment easier and improved the response. The reason to ask first comes from the tretinoin side. MedlinePlus, the U.S. National Library of Medicine's drug guide, says to use only non-medicated cosmetics while using tretinoin, and no other topical medications unless your doctor directs you to. Tretinoin is a retinoid, a family that the American Academy of Dermatology (AAD) says includes retinol, tretinoin, adapalene gel and tazarotene. If you use retinol from a shop, Skinli's page on niacinamide with retinol covers that pair.
Why the prescriber comes first
Tretinoin comes with written rules. MedlinePlus says: "Do not use any other topical medications, especially benzoyl peroxide, hair removers, salicylic acid (wart remover), and dandruff shampoos containing sulfur or resorcinol unless your doctor directs you to do so." It also says: "Use only non-medicated cosmetics on your skin."
The Retin-A label, the official prescribing sheet for one brand of tretinoin, says other topical medication, medicated or abrasive soaps and cleansers, and soaps and cosmetics that have a strong drying effect should be used with caution because of possible interaction with tretinoin. Its patient section says: "Do not use other medications with RETIN-A which are not recommended by your doctor."
Niacinamide appears in neither list. The authors of the 2006 study called their niacinamide product a "barrier-enhancing cosmetic moisturizer." Products vary, though, and some skin care items hold several active ingredients. Your prescriber or pharmacist can read the label of the exact product you want to add and tell you whether it fits your treatment. Keep using the tretinoin as prescribed while you ask.
What the 2006 tretinoin study found
The study, published in the journal Cutis, compared two moisturizers. One contained a mixture of vitamins: niacinamide, panthenol and tocopheryl acetate. The other had similar moisturizing ingredients but no vitamins. The women applied the moisturizers twice daily for 10 weeks. Daily full-face treatment with tretinoin cream 0.025 percent began 2 weeks into the study.
The authors wrote that "improving stratum corneum barrier function before beginning topical tretinoin therapy and continuing use of a barrier-enhancing cosmetic moisturizer during therapy facilitates the early phase of facial retinization and augments the treatment response." The stratum corneum is the outer layer of skin. In plain words, they found that a moisturizer that improves the outer layer of skin, started before tretinoin and kept going during it, made the first phase of tretinoin use easier and improved the result.
The study has three limits that matter for acne:
- The women had sun-damaged skin, which is a different problem from acne.
- The abstract gives no number of participants.
- The moisturizer held three vitamins, so niacinamide alone was not tested.
A second study compared a niacinamide regimen with tretinoin. In 2010, 196 women with wrinkles around the eyes used either a cosmetic regimen with niacinamide, peptides and retinyl propionate or a prescription regimen with tretinoin 0.02 percent for 8 weeks. The authors reported that the cosmetic regimen was better tolerated than tretinoin through 8 weeks by all measures. That study compared the two instead of combining them.
Niacinamide with adapalene, another retinoid
The acne evidence for niacinamide with a retinoid comes from adapalene. A 2024 trial followed 40 people with mild to moderate acne for 8 weeks. Everyone used benzoyl peroxide 5 percent and adapalene 0.1 percent gel. Each person then used a moisturizer with ceramides and niacinamide on one half of the face and a basic cream on the other half. The authors concluded that the niacinamide moisturizer, used with the acne medicines, improved acne lesions and reduced skin irritation. They reported no serious side effects from either cream.
A 2026 review in Skin Health and Disease describes a trial of 88 people using adapalene with benzoyl peroxide, where a cream with niacinamide 2 percent was part of the routine. The review says this shows "the potential for niacinamide in combination with retinoids to improve treatment adherence and, hence, efficacy as the side effects of retinoids can be a treatment barrier." Adherence means keeping to the treatment as planned. Put simply, retinoid side effects make some people stop, and a gentle cream may help them keep going.
Skinli's own plan uses adapalene 0.1 percent gel at night with a light moisturizer. What the guidelines say
The acne guidelines support moisturizer and say little about niacinamide. The AAD says treatments such as adapalene, tazarotene and tretinoin tend to dry and irritate the skin, and that using a moisturizer every day can help your skin tolerate them. The 2024 AAD acne guideline mentions niacinamide only to say that the available evidence is insufficient to develop a recommendation on oral or topical niacinamide for acne treatment. The NICE acne guideline pages and the AAD acne treatment page do not mention niacinamide by name.
A 2025 panel of 62 dermatologists at 43 centers, published in the Journal of the American Academy of Dermatology, listed niacinamide for redness and dark spots and listed retinoids for acne, among other uses. The panel gave no rule on combining them.
How to add niacinamide to a tretinoin routine
No source Skinli read gives an order for niacinamide and tretinoin, or a waiting time between them. These steps come from the sources that do exist:
- Ask first. Take the product, or a photo of its ingredient list, to your prescriber or pharmacist.
- Pick a moisturizer. The studies gave niacinamide in a moisturizer. The AAD says to look for "non-comedogenic", "oil-free" or "won't clog pores" on the label, which means the moisturizer is unlikely to cause acne.
- Apply it after washing. The AAD says applying moisturizer after you wash helps to trap water in your skin.
- Keep tretinoin at night. The AAD says retinoids can make skin more sensitive to sun, so use them only at night and always use sun protection during the day.
- Test on a small spot. The AAD says to apply a new product to a test spot twice daily for seven to 10 days, and to go ahead if there is no red, itchy or swollen skin. Skinli's guide to patch testing has the details.
Add one product at a time. If two new products start on the same day and your skin reacts, you cannot tell which one caused it.
What to keep away from tretinoin
The warnings on tretinoin are about other medicines and harsh products. The Retin-A label says particular caution should be used with preparations containing sulfur, resorcinol or salicylic acid, and advises resting the skin until the effects of such products fade before starting tretinoin. MedlinePlus names benzoyl peroxide and salicylic acid among the medications not to use unless your doctor directs it. Pregnancy, darker skin and when to see a doctor
The AAD says retinoids should not be used during pregnancy. Skinli's guide to retinoids during pregnancy and breastfeeding sets out each source. The AAD also says that with skin of color, irritation can trigger dark marks, known as hyperpigmentation, which gives people with darker skin one more reason to keep irritation low.
Give the routine time. The AAD says it takes at least 6 to 8 weeks before you start to see fewer breakouts, and Skinli's plan says skin needs 8 to 12 weeks to change. Go back to your doctor if acne is painful, forms deep lumps, leaves scars, changes fast, or has not improved after steady care.
