Accutane hair loss and weight changes: the evidence
Does Accutane cause hair loss or weight gain? What the FDA label lists, what studies found, whether hair grows back, and why the dose is set by body weight.

People often search for what isotretinoin, the drug many still call Accutane, does to hair and weight. The short version: the FDA label lists hair loss, and says it has lasted in some cases; it lists weight loss and weight changes, and weight gain is absent from the list. Hair loss: what the label says
The August 2026 FDA label lists alopecia, the medical word for hair loss, among its skin side effects and adds "which in some cases persists." It also lists "hair abnormalities" and hirsutism, meaning extra hair growth. The label does not give a rate for any of these.
The American Academy of Dermatology (AAD) lists thinning hair among the effects that may happen temporarily during treatment, with trouble seeing at night and muscle or joint pain, and says these tend to go away when you stop.
How often it happens
The rates in studies vary widely:

The review authors wrote that they could not test whether the gap between the two dose groups was real, and called for better research. The studies used different doses, lengths of treatment and ways of recording hair loss, which makes their rates hard to compare.
When it starts and whether it grows back
Data on timing are limited. The JAAD International review found only one study that recorded when hair loss started: a median of 4 weeks, with a range of 1 to 24 weeks. The review describes the usual type as telogen effluvium, a form of shedding. It says retinoids are thought to stop hairs from starting a new growth phase and to loosen hairs in their resting phase, which increases shedding.
On recovery, the review found only two of its 22 studies that discussed it. One, a retrospective study, reported that hair loss was temporary in 33 patients who lost hair on high doses. The other commented that hair loss persisted after treatment but, the review says, gave no data to support that. The FDA label's wording, "which in some cases persists," is the official position.
What may raise the risk
A 2022 study from the University of California, Los Angeles, looked back at patients seen for hair loss from 2013 to 2018. Among those whose hair loss started during or within two years after isotretinoin, the patients were older, had taken a larger total amount of the drug, and had taken it for longer than isotretinoin patients from the same years who did not lose hair. The authors suggested that dose or length of treatment may matter, and said the mechanism is still unclear.
The JAAD International review's figures point the same way on daily dose. Both studies are small or look back at records, so neither proves cause.
If your hair is shedding
- Tell your prescriber at the next visit, or sooner if it worries you. The review advises doctors to warn patients about this type of shedding before they start.
- Do not change your dose yourself. The label says the dose may be adjusted for side effects, and that is the prescriber's decision.
- Mention anything else that changed, such as a new medicine, an illness or a diet change, because hair loss has many causes.
- See a dermatologist after treatment if the hair has not recovered, since the label says it persisted in some people.
Weight: what the label says
The label lists weight loss under general side effects, and decreased appetite and weight fluctuation under metabolism. It gives no rate for either. Weight gain does not appear among the side effects. The label does list rises in blood fats and blood sugar changes, which are measured with blood tests rather than on a scale.
What studies found on weight
A 2024 study in the Journal of Cosmetic Dermatology followed 62 patients in Iran who took 20 mg a day or every other day for 3 months. Average weight went from 60.74 kg to 61.12 kg, and the authors found the change could be due to chance. The same study reported rises in LDL cholesterol, triglycerides and blood sugar in some patients, which is why the label asks for blood tests during treatment.
That is one small study, and its dose of 20 mg a day or less is below the label's standard range of 0.5 to 1 mg per kilogram a day for a person of the study's average weight. The label, which draws on clinical trials and reports from patients, remains the main source.
Why the dose depends on body weight
The label gives the dose in milligrams for each kilogram of body weight per day, and the Medication Guide tells patients that the dose "has been specially chosen for you," based on body weight, and "may change during treatment." NICE, which writes guidance for England, also gives a standard daily dose per kilogram and a total amount per kilogram for the whole course. This is why two people can take different amounts, and why weight is recorded at the start. Appetite
The 2022 review of 30 trials counted appetite changes in the few trials that reported them. Decreased appetite appeared in 16.31 percent of patients (23 of 141, in three trial groups), and increased appetite in 3.85 percent (6 of 156, in four trial groups). Those figures come from only a few small trial groups. The label lists decreased appetite; it does not list increased appetite.
Body weight and blood fats
Weight also matters for the blood tests. The label says doctors should consider the risks and benefits with extra care in people at higher risk of high triglycerides, a type of blood fat, and it names obesity and diabetes among those risks. For these patients it recommends more frequent blood fat checks. It also says some patients lowered raised triglycerides while continuing treatment by losing weight and cutting back on dietary fat and alcohol, and others through a lower dose. Questions to ask your prescriber
- Is the hair shedding I see expected at my dose, and will you change anything?
- Should I have my hair checked by a dermatologist after treatment if it does not recover?
- My weight or appetite has changed. Could it be the drug, my mood, or something else?
- Do my weight or health conditions mean I need more frequent blood tests?
When a weight change is a warning sign
A change in weight can point to something other than the drug's direct effect:
- Depression. The Medication Guide lists a change in appetite or body weight among the warning signs. See the Accutane and depression guide.
- Bowel inflammation. The AAD lists belly pain, diarrhea and weight loss among the symptoms of inflammatory bowel disease, and the label says to stop the drug for belly pain, rectal bleeding or severe diarrhea.
- Blood sugar problems. The Medication Guide says to tell your prescriber about unusual thirst or passing urine more than usual.
Exercise during treatment
The label says some patients who did vigorous exercise had raised levels of a muscle enzyme, and that rare cases of serious muscle breakdown have been reported, some linked to hard activity. It warns that teenagers in sports with repeated impact may have a higher risk of certain spine and hip injuries. Tell your prescriber about your training before you start.
